*All measles, mumps & rubella must have been administered on or after the child’s first birthday. **If the 4th dose of DTP and/or 3rd dose of polio are administered on or after the child’s 4th birthday, then the 5th dose of DTP and/or the 4th dose of polio are not required. ***. Exception – Only for students receiving the vaccinations between the ages of 11-15 years of age: 2 doses of RECOMBIVAX HB – given 4 months apart has been approved. The name (RECOMBIVAX HB) and the dosage (1.0ML/CC) must be recorded for the exception to be valid. NOTE: ALL AGE STUDENTS CAN STILL USE THE THREE-DOSE HEPATITIS B SCHEDULE. ****Succeeding years will remain the same as 2010-2011 unless changed by law.
