1. Go to the school assigned to your address. (Refer to the boundary maps to find which school is yours.)
2. Take these documents with you to the school:
a. Proofs of residency - 2 required State Law provides that a child’s residence for school purposes is the school district in which the parent/legal guardian/legal custodian (guardian/custodian must be verified by court documents) holds legal residence. The parent/legal guardian/legal custodian will be asked to provide a current utility bill for gas, electric, or water and sewer (telephone and television cable bills will not be accepted), and a lease agreement, contract on a home (where you will be residing within 45 days), warranty deed statement, or mortgage statement. One proof of residency from each of the two categories must be provided.
b. Legal Birth Certificate Must be the certificate issued BY THE STATE OR COUNTY where the child was born – NOT THE HOSPITAL RECORD. He/she must be five 5 years old on or before September 1, of the current school year to enter kindergarten.
c. IMMUNIZATION RECORD: The State of Oklahoma requires *acceptable evidence of adequate immunization before a child may be enrolled in any school in Oklahoma. Immunization requirements: Elementary, Middle and High Schools.
*Acceptable evidence of adequate immunizations: "Records provided by a licensed physician or public health authority that clearly indicates which immunizations have been received, the dates they were administered, and the signature or stamp of the physician or health clinic that administered the immunizations or interpreted the immunization history." OKLAHOMA SCHOOL IMMUNIZATION LAW.
A previous history of having a vaccine preventable disease is not generally considered grounds for waiving the required immunization. However, measles immunization may be waived if the parent/guardian/custodian presents a signed statement from a licensed physician that indicates the child has had the disease. The Varicella vaccine may be waived if the parent/guardian/custodian presents a signed statement that indicates the child has had the disease. A history of having rubella is not grounds for waiving the immunization.
